Job summary

Brethren Care Village in Ashland, OH seeks a Graduate Practical Nurse (GPN) for Skilled Nursing to join our part-time evening team. The role includes supervising nursing staff, managing resident care, and ensuring adherence to policies.

Onsite presence is required, with evenings and weekend coverage. Pay ranges from $24 to $25.80 per hour, with additional shift differentials for evenings, nights, and weekends based on experience.

Qualifications

• Current and valid CPR certification (or willingness to obtain upon hire).
• Strong leadership skills and/or prior experience preferred.
• Strong customer service skills required.
• Onsite presence required.

Responsibilities

• Maintains nursing care standards and implements department policies and procedures.
• Attends team care conferences as needed.
• Provides oral instructions to nursing assistants.
• Assesses residents and records information in their records.
• Documents administration of medications per policies.
• Functions as team leader and supervisor.
• Interacts with residents, families, and staff to facilitate communication.
• Ensures call lights are answered.
• Receives and records oral orders from physicians.
• Reports changes in health status to physician/responsible party.
• Performs daily tasks following infection control policies.
• Follows body mechanics and lift procedures.
• Ensures care plans are followed by nursing staff.
• Verifies documentation before shift end.
• Ensures rounds occur to prevent conflicts.

Pay: $24/hr to $25.80/hr (Based on experienc e)

PLUS shift differentials: +$1.75/hr evenings, +$1.50/hr nights, +$1/hr weekends)

What You'll Do as a Graduate Practical Nurse (GPN) for Skilled Nursing:

• Maintains the standards of nursing care and implements the policies and procedures of the facilities nursing department including skin grids and treatments, preventative skin plans, preventative fall plans, incident reports and admission paperwork.
• Attends team care conferences as needed.
• Provides oral instructions to nursing assistants.
• Assesses residents and records all pertinent information in resident's records.
• Document administration of medications according to facility policies and procedures.
• Functions as team leader and supervisor.
• Supplies and interprets information, supports decisions, and facilitates communications with residents and their families, and nursing assistants and support staff.
• Assures that resident's call lights are answered.
• Receives and records oral telephone medication and treatment orders from physicians/physician's offices.
• Reports changes in resident's health status to physician, resident, and/or responsible party.
• Performs daily tasks in accordance with infection control and standard precaution policies and procedures.
• Follows policies and procedures related to body mechanics and lift systems.
• Assures residents individual care plans are being followed by reviewing nursing assistant assignment sheets.
• Assures nursing assistant are completing their required documentation before shift ending including service check off lists, Restorative Care and Intake and Output records.
• Assures rounds are being done with shift to shift change to prevent conflict

Requirements for a Graduate Practical Nurse (GPN) for Skilled Nursing?

• Current and Valid CPR certification or willingness to obtain upon hire required.
• Strong leadership skills and/or previous experience are preferred.
• Strong Customer Service skills required.
• Onsite presence required.
