Hartford HealthCare is seeking a Graduate Nurse (RN) for the Nursing Float Pool at St. Vincent's Medical Center in Bridgeport, CT. This full-time role supports direct patient care and care coordination in a dynamic, interdisciplinary environment.

Ideal candidates are graduates from an accredited nursing program with CT RN licensure and BLS certification. Associate degree is required; BSN preferred. The position offers residency opportunities and comprehensive benefits.

Qualifications

• Graduate of an accredited nursing school.
• Associate degree required; BSN preferred.
• Current Connecticut RN license.
• BLS certification by the American Heart Association.

Responsibilities

• Implement and monitor patient care plans.
• Monitor, record, and communicate patient condition as appropriate.
• Serve as a primary coordinator of all disciplines for well-coordinated patient care.
• Note and carry out physician and nursing orders.
• Assess and coordinate patient's discharge planning needs with the healthcare team.
• Delegate aspects of care to other nursing personnel.
• Participate in daily interdisciplinary patient rounds.
• Develop, implement, and revise individualized plans of care.
