Qualifications

Responsibilities

• Implement and monitor patient care plans.
• Coordinate, monitor and communicate patient condition with the care team.
• Coordinate discharge planning with the healthcare team.
• Delegate aspects of care to other nursing personnel and participate in rounds.
• Develop, implement, and revise individualized plans of care.
• Engage in professional growth activities outside direct patient care.
