Hartford HealthCare in Bridgeport, CT is seeking a Graduate Nurse (RN) for Acute Rehab at St. Vincent’s Medical Center. This full-time, day-shift role offers a supportive environment for new graduates to start their nursing careers.

Responsibilities include implementing care plans, coordinating multidisciplinary care, assisiting with discharge planning, and participating in daily rounds. Requires an associate degree; BSN preferred, and CT RN license with BLS.

Qualifications

• Education requirements include an associate degree; BSN preferred.
• Graduate of an accredited nursing school.
• Current Connecticut RN license and BLS certification.

Responsibilities

• Implement and monitor patient care plans.
• Monitor, record, and communicate patient condition as appropriate.
• Serve as a primary coordinator of all disciplines for well-coordinated patient care.
• Note and carry out physician and nursing orders.
• Assess and coordinate patient's discharge planning needs with the healthcare team.
• Delegate aspects of care to other nursing personnel.
• Participate in daily interdisciplinary patient rounds.
• Develop, implement, and revise individualized plans of care.
• Participate in activities for professional growth and development.
