Job summary

Veterans Transition Center of California seeks a GPD Program Manager to lead the transitional housing program and supervise the case management and intake teams. You will oversee daily operations, staff performance, and grant administration to help Veterans obtain permanent housing and stability.

The role includes coordination with the VA GPD liaison, development of individualized service plans, and ensuring accurate documentation and reporting across HMIS systems.

Qualifications

• Master’s degree in social work or related field required.
• Experience supervising staff and coordinating case management.
• Experience with grant administration and reporting.
• Strong documentation and record-keeping skills.
• Knowledge of crisis response and trauma-informed care is preferred.

Responsibilities

• Supervise four Case Managers and one Intake Coordinator; oversee caseloads and evaluations.
• Coordinate intake and housing planning with VA GPD liaison.
• Provide participant support and coordination of services, including healthcare and benefits.
• Ensure compliance with grant requirements and accurate HMIS data entry.
• Maintain safety procedures and staff readiness for crisis situations.
