Job summary

Oak Street Health in Fort Wayne, IN is seeking a Primary Care Physician to deliver equitable value-based healthcare for Medicare patients. You will work within a neighborhood center supported by a care team to improve outcomes and patient experience.

Qualifications

• MD or DO degree required with board certification in internal or family medicine.
• Active, unrestricted State Medical License and DEA registration.
• US work authorization is required.
• Fellowship in geriatrics is preferred with geriatric clinical experience.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care services.
• Prescribe medications and manage therapies.
• Maintain EMR data using Canopy and Greenway systems.
• Collaborate with leadership to meet health quality goals.
• Coordinate with Practice Managers to direct center care teams.
• Perform other duties as assigned.

Education

MD or DO degree

Board Certification in Internal Medicine or Family Medicine

Active State Medical License

Active DEA license

US work authorization

Fellowship in Geriatrics

Geriatric medicine experience

Experience in Value-Based Healthcare

Outpatient primary care experience

Bilingual (where applicable)
