Oak Street Health in Fort Wayne, IN is seeking a Primary Care Physician to deliver equitable, value-based care for local Medicare populations. The role focuses on quality of care over volume within a neighborhood center supported by a care team.

New graduates welcome; experienced physicians may be considered for advanced leadership roles. Competitive base salary with bonuses and a full benefits package. 40 hours per week, full-time schedule.

Qualifications

• Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) graduate.
• Internal Medicine or Family Medicine Board Certification (Or board eligible).
• Active, non-probationary, unrestricted State License.
• Active DEA license.
• US work authorization.

Responsibilities

• Assess and diagnose patients at our local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and administer pharmaceutical treatments and medication.
• Maintain Patient Electronic Medical Record data via canopy and greenway.
• Collaborate with regional and central leadership to meet health quality goals.
• Work with Practice Managers to direct and manage the center care team.
• Other duties as assigned.

Education
