Qualifications

• One year of post-graduate clinical experience with geriatrics preferred.

Responsibilities

• Provide care in outpatient clinics and community-based settings.
• Collaborate with physicians, NPs, social workers, pharmacists, and therapists.
• Deliver comprehensive, whole-person care across medical, behavioral health, and supportive services.
