Job summary

Providence PACE is seeking a full-time Physician to join our Portland, Oregon team. You will provide primary care across diverse settings, including outpatient clinics and home/community visits, while collaborating with an interdisciplinary team to support older adults’ health and independence.

The ideal candidate has experience with geriatric populations and a strong team-based approach; training in palliative care or hospice is a plus.

Qualifications

• Medical degree and license to practice in Oregon
• Experience caring for older adults in primary care or geriatric settings
• Ability to work in an interdisciplinary team and across multiple sites

Responsibilities

• Provide primary care across multiple community-based settings and clinics
• Collaborate with physicians, nurse practitioners, care managers, social workers and pharmacists
• Participate in rotating coverage across seven Providence PACE centers
• Support continuity of care and care transitions for patients in diverse care settings
