Job summary

Salgado-LLC is seeking a Physician to provide direct primary medical care within an Interdisciplinary Team framework, supervising PCP Nurse Practitioners, RNs, and Certified Medical Assistants. You will perform histories, physicals, and care coordination for geriatrics, including nursing home patients, with regular interdisciplinary communication.

You will participate in family conferences, quality improvement, and ethics initiatives to advance geriatric care standards.

Qualifications

• Valid MD/DO license in good standing to practice medicine in the State.
• Graduate of an accredited School of Medicine.
• Board certification.
• Minimum of two years of clinical experience.
• Experience or training in working with frail or elderly populations.
• Knowledge of geriatric medicine and the special needs of complex geriatric patients.
• Medically cleared for communicable diseases and up-to-date immunizations.
• Ability to pass a criminal background check, including verification of eligibility to participate in Medicare and Medicaid programs.

Responsibilities

• Provide direct primary medical care to patients within the Interdisciplinary Team.
• Supervise PCP Nurse Practitioners, RNs, and Certified Medical Assistants.
• Perform comprehensive history and physical examinations on new referrals.
• Coordinate hospital admissions and discharge planning with the IDT.
• Manage care for patients in nursing homes with regular visits and communication with nursing home staff.
• Participate in family conferences for care planning.
• Document patient changes in medical records and communicate updates to the IDT.
• Actively participate in IDT meetings and contribute to care planning and quality improvement committees.
