Job summary

Salgado-LLC is seeking a Physician to provide direct primary medical care to patients and supervise medical care personnel within an Interdisciplinary Team (IDT) framework in Fresno, CA. The role includes elderly patient management, family conferences, and coordination of hospital admissions.

The position requires an MD/DO in good standing, board certification, and at least two years of clinical experience with frail elderly populations. Strong geriatric knowledge and immunizations are essential.

Qualifications

• MD/DO license in good standing in the state of practice.
• Graduate of an accredited School of Medicine.
• Board certification in relevant specialty.
• Minimum of two years of clinical experience.
• Experience with frail or elderly populations.
• Knowledge of geriatric medicine and complex geriatric needs.
• Immunizations up-to-date and medically cleared.
• Ability to pass criminal background check and eligibility for Medicare/Medicaid.

Responsibilities

• Provide direct primary medical care to patients within the Interdisciplinary Team (IDT).
• Supervise medical care personnel (PCP NPs, RNs, CMAs) within the team.
• Perform histories and physical examinations on new referrals and established patients.
• Coordinate hospital admissions and discharge planning with IDT.
• Manage care for patients in nursing homes and communicate with staff.
• Participate in family conferences and care planning discussions.
• Document changes in medical records and update IDT.
• Participate in IDT meetings and contribute to quality care planning.
• Contribute to ethics and quality improvement initiatives.
