Job summary

Salgado-LLC seeks a Physician to deliver direct primary care and supervise healthcare staff within the Interdisciplinary Team framework. You will perform histories, annual exams, and episodic care for geriatric patients, coordinating hospital and nursing home care as needed.

Responsibilities include care planning with families, documentation in medical records, and active participation in Ethics and Quality Improvement efforts. Board-certified physicians with geriatric experience are preferred.

Qualifications

• MD/DO license valid in good standing to practice in the state.
• Graduate of an accredited School of Medicine.
• Board certification.
• Minimum of two years of clinical experience.
• Experience with frail or elderly populations.
• Knowledge of geriatric medicine and needs of complex geriatric patients.
• Immunizations up-to-date and cleared for communicable diseases.
• Ability to pass a criminal background check and Medicare/Medicaid eligibility.

• Perform comprehensive history and physicals on new referrals.
• Conduct interval histories and physicals for established patients.
• Evaluate and treat episodic illnesses.
• Refer patients to medical specialists as needed.

• Manage care for nursing home residents and communicate with staff.
• Participate in family conferences for care planning.
• Document patient changes in medical records and share updates with the IDT.
• Attend IDT staff meetings and contribute to care planning.
• Maintain and enhance education on quality standards and geriatric best practices.
• Serve on Ethics and Quality Improvement committees or work groups.
• Other duties as assigned

Job description

Overview:
Our client is seeking a Physician to provide direct primary medical care to patients and supervise medical care personnel, including PCP Nurse Practitioners, RNs, and Certified Medical Assistants. The Physician will deliver care within the Interdisciplinary Team framework.

Perform comprehensive history and physical examinations on new referrals.

Conduct interval history and physicals on established patients quarterly or as indicated.

Evaluate and treat patients with episodic illnesses.

Refer patients to medical specialists as needed.

Coordinate hospital admissions and discharge planning with the IDT.

Manage care for patients residing in nursing homes, including regular visits and communication with nursing home staff.

Actively participate in family conferences for care planning.

Document patient changes in medical records and communicate updates to the IDT.

Participate as a member of the IDT, attending staff meetings and contributing to care planning.

Maintain and enhance education regarding quality clinical standards and best practices for geriatric care.

Serve on Ethics and Quality Improvement Committees or work groups as assigned.

Other duties as assigned
