Job summary

Salgado-LLC in California seeks a Physician to provide direct primary medical care to patients and supervise medical care personnel within the Interdisciplinary Team (IDT). The ideal candidate holds MD/DO, board certification, and at least two years of clinical experience, with a strong background in geriatric medicine and patient-centered care.

Qualifications

• Valid MD/DO license in good standing to practice medicine in the State.
• Board certification in medicine.
• Minimum of two years of clinical experience.
• Experience or training with frail or elderly populations.

Responsibilities

• Perform comprehensive history and physical examinations on new referrals.
• Conduct interval history and physicals on established patients quarterly or as indicated.
• Evaluate and treat episodic illnesses.
• Coordinate hospital admissions and discharge planning with the IDT.
• Manage care for patients in nursing homes with regular visits.
