Job summary

Qualifications

• Doctorate in Medicine from an ACGME-accredited residency program.
• Board Certification in relevant fields is preferred.
• Must have an active California Medical License.

Responsibilities

• Conduct in-person comprehensive assessments for patients.
• Coordinate care plans with the Interdisciplinary Team.
• Oversee care for patients in nursing homes and hospitals.
• Monitor transitions of care including admits and discharges.
• Guide and supervise Nurse Practitioners as needed.
• Participate in rotating night and weekend call coverage.
