Qualifications

• Medical degree and licensure in Massachusetts.
• Board certification in Internal Medicine, Geriatrics, or Family Practice preferred.
• Active DEA certificate and driving/transportation as essential function.

Responsibilities

• Provide routine, preventive and urgent care to PACE participants.
• Conduct initial assessments and biannual re-evaluations with IDT.
• Develop and maintain participant care plans and coordinate specialist reports.
• Complete charting and documentation for revenue and care planning.
• Participate in after-hours on-call arrangements and committees as requested.
