Primary Care Physician (PACE) – Springfield, MA

Job Type: Full-time, onsite

Comp Range: $280,000–$300,000

Reports To: Medical Director

This is a PACE Primary Care Physician (MD/DO) role caring for a frail, older adult population enrolled in the PACE program. It’s a team-based, relationship-driven model — not fee-for-service, not RVU-heavy, and not traditional clinic “volume churn.”

Ideal for physicians looking for work-life balance, manageable panel sizes, true continuity of care, and an interdisciplinary environment where quality > speed.

Must-Haves

• MD or DO — Board Certified or Board Eligible in Internal Medicine or Family Medicine
• Strong comfort with: Complex geriatric medical needs; Behavioral + psychosocial issues; Chronic disease management; Primary care; Geriatrics (preferred); Community health, FQHC, VA, home-based medicine, SNF, PACE, or hospice
• Desire to work in a PACE environment: low-volume, high-acuity, interdisciplinary
• Clear communication skills; team-first attitude
• Organizational maturity; able to collaborate with IDT (nursing, PT/OT, social work, behavioral health, transportation, etc.)
• Required(Currently): Bilingual Russian

Preferred but Not Required

• Experience with: AMR/Epic/other EMR transitions; Interdisciplinary team collaboration; Home visits; Community-based clinical navigation (Day Center + Participant homes)

The physician must be comfortable with:

• Providing primary medical care to a frail older adult population
• Conducting assessments and developing care plans in collaboration with the IDT
• Participating in daily huddles or IDT rounds
• Aligning medical plans with social, mental health, and functional needs
• Performing procedures within scope
• Occasional home visits or in-center visits
• Evaluating care plans for effectiveness and adjusting as needed
• Working collegially with NP/PA, RN, PT/OT, Social Work, Dietician, and Transportation staff
