Job summary

Fallon Health Summit ElderCare in Worcester, MA is seeking a SNFist physician to join the PACE program. This role focuses on high-quality, patient-centered care in skilled nursing facilities, with 5–6 patients per day and a Monday–Friday schedule.

You will work with an interdisciplinary team, provide medical oversight, and lead care transitions from hospital to SNF while supporting the goal of helping frail elders live safely at home. Salary ranges from $285,000 to $310,000 per year.

Qualifications

• MD or DO licensed in Massachusetts.
• Excellent communication and interpersonal skills.
• Demonstrated ability to work in an interdisciplinary team model.
• Proficiency in using electronic medical records (EMR).

Responsibilities

• Review and cosign interdisciplinary documentation to ensure accuracy and regulatory alignment.
• Admission H&P, physician visits, medications and treatment plans review, orders and progress notes.
• Manage and coordinate treatment for participants with multiple chronic conditions.
• Lead transitions from hospital to SNF and ensure timely care.
• Conduct daily rounds in assigned SNFs and collaborate with SNF staff.
• Coordinate with SE IDT members for coordinated care.
• Communicate with participants, caregivers, facility staff and the IDT about goals and plans.
• Maintain presence in SNFs to promote physician accessibility and adherence to PACE principles.
