Job summary

Fallon Health is seeking a SNFist for the Summit ElderCare PACE program in Worcester, MA. The physician will oversee care in SNFs, coordinate with an interdisciplinary team, and support frail elders to live safely at home.

Qualifications

• Medical degree (MD or DO) from an accredited school of medicine.
• MD or DO licensed in Massachusetts.
• Board Certification in Internal Medicine, Family Practice, or Geriatrics (preferred).
• DEA certification.
• 1+ year experience with frail or elderly populations.
• Passion for team-based, patient-centered care.
• 2-3 years’ experience in post-acute care, long-term care or geriatric settings.
• Strong clinical skills in managing complex, frail patients with multiple chronic problems.
• Knowledge of SNF regulations and standards of care preferred.
• Excellent communication and interpersonal skills.
• Ability to work effectively in an interdisciplinary team model (PACE preferred).
• Proficiency in using electronic medical records (EMR).

Responsibilities

• Clinical care and oversight in SNFs and PACE settings.
• Review interdisciplinary documentation for accuracy and regulatory alignment.
• Admission H&P, physician visits, medications, and care plans review.
• Coordinate treatment for participants with chronic conditions and polypharmacy.
• Lead transitions hospital to SNF to ensure timely care.
• Conduct daily rounds in assigned SNFs.
• Collaborate with SNF nursing, rehab teams, and Directors of Nursing.
• Participate in SE IDT meetings and coordinate care with the team.
• Communicate goals and plans with participants and caregivers.

Education

MD or DO licensed in Massachusetts
