Qualifications

• Medical degree, Geriatric specialty strongly desired.
• Licensed MD/DO in Massachusetts with DEA certificate.
• Board certification Internal Medicine, Geriatrics or Family Practice preferred.
• Valid driver's license and reliable transportation required.
• Experience with frail/elderly population preferred.

Responsibilities

• Support intake and enrollment and conduct initial assessments.
• Conduct biannual participant assessments.
• Develop and maintain participant care plans.
• Provide routine, preventive and urgent care.
• Collaborate with the interdisciplinary team (IDT).
• Document full diagnostic and procedure codes for care plans.
• Participate in after-hours on-call as needed.
• Represent Summit ElderCare at external meetings.
