Job summary

Pine Park Health Management Inc in Fremont, CA seeks a full-time Nurse Practitioner to deliver care in the East Bay San Francisco market (Oakland, Walnut Creek, Concord). Join a value-based primary care team serving senior living communities with a smaller patient panel and a multidisciplinary approach.

You’ll develop person-centered care plans, participate in CCM visits, and lead clinical staff while traveling between partner communities. Hybrid in-office and remote scheduling offered.

Qualifications

• Nurse Practitioner with geriatrics experience.
• Willingness to provide onsite clinical across East Bay SF region.
• Desire to work with complex care patients and holistic care model.
• Strong collaboration with multidisciplinary clinical team.
• Strong written and verbal communication skills.
• Experience with tech-enabled care delivery tools.
• Flexible travel within East Bay.

Responsibilities

• Provide onsite primary care services to older adults living in Senior Living Communities.
• Develop and execute detailed person-centered care plans for primary care patients.
• Work with Medical Director to identify and develop policies affecting day-to-day practice.
• Provide CCM visits and review CCM documentation.
• Run regular huddles and provide leadership to the clinical team.
• Collaborate with senior living management to implement Pine Park care plans and improve outcomes.
