Job summary

Pine Park Health is hiring a Full-Time Nurse Practitioner to deliver onsite primary care to older adults in East Bay communities (Oakland, Walnut Creek, Concord). The role focuses on patient-centered care within a value-based model and requires coordination with a multidisciplinary team.

The NP will develop detailed care plans, perform CCM visits, lead clinical huddles, and travel between partner communities in the East Bay San Francisco region.

Qualifications

• Nurse Practitioner license required, geriatrics experience preferred.
• Ability to work in fast-paced environment and adapt to changes.
• Willingness to provide onsite clinical across East Bay San Francisco region.
• Desire to support holistic, patient-centered care in senior living communities.
• Strong collaboration with multidisciplinary team and effective communication.

Responsibilities

• Provide onsite primary care to older adults in Senior Living Communities.
• Develop and execute detailed patient-centered care plans for primary care patients.
• Collaborate with Medical Director to address day-to-day practice issues and clinical guidelines.
• Conduct CCM visits and review CCM documentation monthly.
• Lead clinical team through regular huddles and coordinate with senior living management.
