Carease Health is seeking an experienced Nurse Practitioner to deliver preventive and chronic disease care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for documentation.

Responsibilities include assessments, care plans, medication management, and collaboration with facility staff to ensure comprehensive care.

Qualifications

• Active NP license in state of practice (Tennessee).
• 1+ year of experience in geriatric care or internal medicine.
• Excellent communication, collaboration, and teamwork skills.
• Patient-centered approach with ability to build relationships with patients and facility staff.

Responsibilities

• Provide advanced nursing care to residents in skilled nursing facilities, focusing on chronic disease management and prevention.
• Conduct patient assessments, develop care plans, and provide medication management and education.
• Collaborate with facility staff, families, and other healthcare professionals to ensure comprehensive care.
• Utilize Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) to proactively manage patient care.
• Conduct on-site visits and remote work from home, with support from our dedicated scribe team.
• Maintain accurate and timely documentation for seamless patient transitions and facility communication.
• Participate in quality improvement initiatives to enhance patient outcomes and facility performance.
