Vitae Health Systems is seeking a dedicated Nurse Practitioner to deliver high-quality medical care to skilled nursing facility residents. You will assess, diagnose, and manage acute and chronic conditions in collaboration with an interdisciplinary team to improve patient outcomes and quality of life.

The role emphasizes value-based care, documentation accuracy, and ongoing professional development within a supportive leadership structure.

Qualifications

• Master’s degree in Nursing from an accredited program.
• Active and unrestricted APN license in the relevant state.
• Current DEA registration and prescriptive authority (or eligibility).
• Board certification as a Nurse Practitioner (e.g., FNP, AGNP).
• 2 years of clinical experience, preferably geriatrics or long-term care.

Responsibilities

• Perform comprehensive health assessments and develop care plans for residents.
• Provide ongoing medical management, order/interpret diagnostics, and adjust treatments.
• Deliver timely interventions to improve outcomes and reduce hospitalizations.
• Collaborate with physicians and the care team for coordinated care.
• Educate residents and families on diagnoses, plans, and prevention.
• Maintain compliant documentation per standards and regulations.
