Job summary

The Ohio State University Physicians, Inc. seeks a Family Nurse Practitioner or Physician Assistant to join the Geriatrics team in skilled nursing facilities.

The APP serves as a primary care provider during residents' stays, managing medications, arranging imaging and labs, and planning discharges for post-acute and long-term care patients. Responsibilities include coordinating care with physicians, reviewing patient medications, and contributing to a rotating call schedule.

Qualifications

• NP or PA credentials with prescriptive authority.
• Experience in primary care or geriatric settings preferred.
• Ability to educate patients and families about care plans.

Responsibilities

• Performs history and physical examinations on new and return patients; establishes presumptive diagnosis; orders labs and tests.
• Works with physician to formulate treatment plans for health problems and follow-up.
• Reviews medications and develops patient care plans within nursing protocols.
• Evaluates outcomes and adjusts nursing approach accordingly.
