The AP provider will deliver care to patients and families, perform assessments and management, collaborate with physicians, maintain medical records, and support health promotion and disease prevention.

Graduate from master's NP program or Accredited PA program; 1-3 years clinical experience preferred.

Qualifications

• Provides assessment, evaluation, and management to patients.
• Maintains and reviews patient records in the electronic medical record.
• Collaborates with physicians and other health professionals to develop care plans.

Responsabilités

• Delivers health care services to patients, families and groups under supervision.
• Includes follow-up care, test results, and health counseling as part of patient management.
• Orders appropriate diagnostic tests and prescribed pharmacological treatments.
• Maintains comprehensive documentation and coordinates with the care team.
