Matter Health seeks a Nurse Practitioner to deliver primary and preventive care in Nashville's community housing settings. You will own a patient panel, provide ongoing care, and collaborate with an interdisciplinary team to improve access and outcomes.

The role emphasizes autonomy in care delivery, in-home visits, and telehealth, with emphasis on equitable, patient-centered services and robust EHR documentation.

Qualifications

• Active, unrestricted Tennessee NP license.
• Tennessee Certificate of Fitness to prescribe.
• Board certification as NP through AANPCB or ANCC (family, adult, geriatric, or adult-gerontology).
• Active DEA registration.

Responsabilités

• Provide ongoing primary and preventive care to adult patients in affordable housing communities.
• Manage and grow a dedicated patient panel within assigned communities.
• Deliver relationship-driven, patient-centered care in community and in-home settings.
• Collaborate with interdisciplinary clinical and operational teams.
• Support preventive care, chronic disease management, and care coordination.
• Use telehealth and in-person care to improve accessibility; maintain accurate EHR documentation and coding specificity.
