Job summary

Matter Health is hiring a Nurse Practitioner to provide community-based primary care in Nashville, Tennessee. You will own a dedicated patient panel and deliver ongoing care in affordable housing communities, with strong collaboration from an interdisciplinary team.

The role emphasizes autonomous practice with support from a Medical Director, focusing on preventive care, chronic disease management, and care coordination, including telehealth options and in-home visits where appropriate.

Qualifications

• Active Tennessee NP license or eligibility prior to start.
• Tennessee Certificate of Fitness to prescribe before start date.
• Board certification as NP (AANPCB/ANCC) in family/geriatric/adult.
• DEA registration active or obtain prior to start date.
• Minimum 3+ years NP clinical experience with adults/geriatric patients.
• BLS certification valid at start date.
• Valid driver's license with reliable transportation; travel stipend provided.
• Experience with EHR documentation, preferably athenahealth.
• Commitment to equitable, patient-centered care.

Responsibilities

• Provide ongoing primary and preventive care to adults in affordable housing communities.
• Manage and grow a dedicated patient panel within assigned communities.
• Deliver patient-centered care in community and in-home settings.
• Collaborate with interdisciplinary clinical and operational teams.
• Support preventive care, chronic disease management, and care coordination.
• Utilize telehealth and in-person visits to improve access.
• Maintain accurate EHR documentation with coding specificity.
