Community LIFE in Forest Hills, PA seeks an experienced Nurse Practitioner or Physician Assistant to serve as Advanced Practice Provider Supervisor, a leadership role combining clinical practice with team development. This position covers all service areas with local travel.

You will lead the APP team, promote clinical excellence, collaborate with physicians and interdisciplinary staff, support recruitment and onboarding, provide direct patient care when needed, and drive care planning, QA, and

Competenze

• Master's or Doctoral NP program or PA program graduate
• 5+ years as NP/PA in clinical practice
• 3+ years of leadership or supervisory experience

• Lead, mentor, and support the APP team.
• Promote a positive team culture and clinical excellence.
• Collaborate with physicians and interdisciplinary team members to ensure high-quality participant care.
• Support recruitment, onboarding, and professional development of APP staff.
• Provide direct participant care and coverage as needed.
• Conduct assessments and manage chronic and acute conditions; coordinate specialty referrals.
• Participate in care planning, quality improvement initiatives, and regulatory compliance activities.
• Work Monday–Friday, 7:30am–4:00pm and provide limited on-call coverage in rotation.
