Community LIFE in Forest Hills, PA seeks an experienced Nurse Practitioner or Physician Assistant to serve as Advanced Practice Provider Supervisor, a leadership role combining clinical practice with team development. This position covers all service areas with local travel.

Community LIFE is a Program of All-Inclusive Care for the Elderly (PACE) dedicated to helping older adults live safely and independently in their homes and communities.

We are seeking an experienced Nurse Practitioner or Physician Assistant to serve as our Advanced Practice Provider Supervisor. This NEW leadership role combines clinical practice with team development, supporting APPs across our service areas while helping older adults remain independent and healthy in their communities. This position will be based out of our Forest Hills, PA (15221) location but will cover all of our service areas, so will require local travel with mileage reimbursed. What You'll Do

Required Education

• Nurse Practitioner: Master's, Post-Master's, or Doctoral NP program
• Physician Assistant: Graduate of an accredited PA program

Required Experience

• 5+ years as a Nurse Practitioner or Physician Assistant
• 3+ years of leadership or supervisory experience
• Experience with geriatrics, aging services, or PACE preferred

Required Licensure

• Current Pennsylvania NP or PA license
• Prescriptive Authority and DEA registration
• Valid Pennsylvania driver's license
• All licenses must be active and unrestricted
