Job summary

Suvida Healthcare in Austin, TX is seeking an Advanced Practice Provider to lead outpatient care within our Neighborhood Center. The APP will diagnose, manage, and coordinate care in a multidisciplinary, value-based model for Medicare-eligible seniors, under the supervision of the Medical Director.

Ideal candidates have geriatric experience, active Texas or Arizona license, DEA license, and bilingual English/Spanish abilities, with strong leadership and patient-focused service.

Qualifications

• Active, unrestricted medical license in Texas or Arizona.
• Board certification or eligibility in Family, Adult, and Geriatrics.
• Current DEA license.

Responsibilities

• Cares for Suvida Healthcare’s patients in outpatient medicine and preventive care.
• Manages patients under supervision of Neighborhood Center Medical Director.
• Leads their care team within the Neighborhood Center.
• Collaborates with the multidisciplinary care team.
• Attends daily morning huddle to discuss patients and close care gaps.
• Attends weekly huddle to review high-risk patients and plan care.
