Suvida Healthcare LLC in Houston, TX seeks a Neighborhood Center Physician to drive outpatient medicine within a value-based care model, lead a multidisciplinary care team, and deliver 5-Star care to Medicare-eligible Hispanic seniors.

You will partner with the Neighborhood Center Medical Director, participate in daily and weekly huddles, manage complex cases, and support health equity by tailoring care to culture and language.

Formación

• 3+ years minimum experience, preferably in geriatrics.
• Active, unrestricted medical license in Texas (or Arizona)
• Board certification or eligibility in Family or Internal Medicine
• Current DEA license
• Bilingual in English and Spanish preferred

Responsabilidades

• Cares for patients in outpatient medicine focusing on prevention and outcomes.
• Manages patients in a multidisciplinary value-based care model.
• Leads the Neighborhood Center care team.
• Collaborates with multidisciplinary care team.
• Provides high level of customer service and 5-star care.
• Keeps complete medical history and records.
• Refers patients to specialists as needed.
• Attends daily morning huddle with care team; addresses care gaps.
• Attends weekly huddle to review high-risk patients.
• Influences department strategy and decisions.
• Oversees Advanced Practice Providers as needed.
• Reports to Neighborhood Center Medical Director
