Responsibilities

• Cares for patients in outpatient medicine, focusing on continuity of care and disease prevention.
• Manages patients in a multidisciplinary value-based care model.
• Leads their care team within the Neighborhood Center.
• Collaborates with multidisciplinary care team to close care gaps.
• Maintains medical history and records and refers patients to specialists as needed.
• Attends daily and weekly huddles to discuss patient care and high-risk patients.
