Job summary

Eaton County Health & Rehabilitation Services is seeking an Admissions Coordinator (Non-Clinical) to manage hospital referrals for rehab and long-term care, communicate with discharge planners, and ensure timely collection of referral data.

The role includes entering patients into the EMR, daily follow-ups on pending referrals, completing admission paperwork, and coordinating with the care team. Knowledge of insurance, EMR, and Microsoft Office is preferred.

Qualifications

• Must have genuine interest in geriatrics and assuring quality medical records of residents.
• Ability to adapt to frequent changes in routine.

Responsibilities

• Review all referrals submitted by hospitals for admission to rehab and/or LTC.
• Open communication with hospital discharge planners on acceptance or denial of patient referrals.
• Ensure required referral information and data are obtained timely prior to an admission arriving.
• Entering patients in the EMR.
• Follow up daily on all pending referrals.
• Completion admission paperwork.
• Bridge communication with the team regarding new admissions.
• Basic understanding of insurance, electronic medical records and medical terminology preferred.
• General use of Microsoft Office Tools.
