Qualifications

• Graduate from an accredited school of practical nursing.
• Current nursing license and state registration (LPN or RN).
• Minimum three years of geriatric nursing experience.

Responsibilities

• Assist in coordination of the resident admission process including screening, assessment, and care needs
• Ensure care is delivered as indicated in resident care plans
• Medication and treatment administration according to physicians’ orders
• Coordinate care with outside providers including resident physicians
• Document and report care provided and resident observations
• Maintain knowledge of state regulations, policies, and procedures
