Job summary

Asbury Communities, Inc. is seeking a full-time Director of Wellness (LPN) at our 27-bed personal care home in York, PA. The role leads wellness staff, ensures resident-centered care, and oversees medication management while aligning with regulatory standards. Night/weekend/holiday availability is required.

Qualifications

• Graduate of an accredited school of nursing with PA nursing license.
• Current LPN license in good standing in Pennsylvania.
• Current CPR certification.

Responsibilities

• Plan, assign and direct wellness workflow and staff with training and procedures.
• Act as Manager on Duty as scheduled.
• Pre-screen residents to assess care needs and greet new residents; develop DME/RASP docs per state regs.
• Maintain medication management protocols to ensure safety and regulatory compliance.
• Communicate effectively with health care providers, agencies and families.
