Habitat Health is seeking a Functional Wellness Supervisor to lead the rehab team at our PACE center in California. The role focuses on high‑quality, participant‑centered rehab services, coordinating therapy programs and aligning with CMS and state standards.

You will drive performance, data-driven improvements, and cross‑department collaboration to support independence and safety. You will supervise PT/OT and alignment of functional wellness programming, while fostering a welcoming, inclusive

Qualifications

• Unrestricted OT or PT license in state of practice, or eligible, with bachelor’s degree.
• 2–4 years of experience, with at least 1 leading people.
• Geriatric rehab experience and functional assessments.
• Strong knowledge of PACE regulations, CMS requirements, and licensure standards.
• Experience developing functional wellness programs and fall-prevention initiatives.
• Ability to interpret clinical and operational data to drive improvements.
• Collaborative, patient-centered care planning and strong communication.

Responsibilities

• Lead rehabilitation and functional wellness services at the Center to support independence, safety, and quality of life.
• Oversee Occupational Therapy, Physical Therapy, and integrated functional wellness programming.
• Align clinical operations with CMS regulations, state licensure standards, and organizational goals.
• Partner with interdisciplinary team and center leadership to ensure evidence-based interventions and care plans.
• Manage staffing, scheduling, and workflows; coach performance and ensure coverage.
• Monitor KPIs (utilization, functional outcomes, patient satisfaction) and drive quality improvement via audits and corrective actions.
• Build a data-driven practice by standardizing outcome measures and reporting dashboards.
• Support efforts to reduce preventable hospitalizations through proactive interventions.
• Lead fall prevention and recovery initiatives; risk screening and coordination with nursing and providers.
• Champion innovation by piloting programs/tools and scaling best practices across centers.
• Emphasize functional wellness to reduce acute utilization and aging-in-place.
