Job summary

Nexum HC in Chicago, IL is seeking a licensed clinical social worker (LCSW) or licensed clinical professional counselor (LCPC) to provide psychotherapy in a skilled nursing and rehabilitation facility. This full-time, in-person role offers meaningful work with older adults.

You will conduct assessments, develop treatment plans, deliver individual and group therapy, and collaborate with nurses, physicians, and social services to coordinate care.

Qualifications

• Master's degree in Social Work, Counseling, Psychology, or a related field.
• Active Illinois licensure as an LCSW or LCPC.
• Strong clinical assessment, treatment planning, and documentation skills.
• Excellent communication and interpersonal skills.
• Ability to work independently while collaborating with a multidisciplinary team.
• Experience in nursing homes/healthcare settings preferred.
• Experience providing individual and group therapy preferred.

Responsibilities

• Conduct comprehensive assessments and diagnostic evaluations to identify mental health needs.
• Develop and implement individualized treatment plans based on client goals and clinical best practices.
• Provide individual and group psychotherapy utilizing evidence-based therapeutic interventions.
• Facilitate group therapy sessions as appropriate.
• Maintain accurate and timely clinical documentation in compliance with legal and ethical standards.
• Collaborate with nursing staff, physicians, social services, and other healthcare professionals to coordinate care.
• Support residents experiencing depression, anxiety, grief, adjustment concerns, and other behavioral health needs.
• Promote emotional wellness and improve quality of life for residents.
