Job summary

Nexum HC is seeking a compassionate LCSW, LCPC, or LMFT to provide psychotherapy within a skilled nursing and rehabilitation facility in Oak Park, Illinois. The role involves conducting assessments, creating treatment plans, and delivering individual and group therapy to older adults onsite.

The ideal candidate holds a Master’s degree in a related field, active Illinois licensure, and strong documentation and collaboration skills to work with a multidisciplinary team.

Qualifications

• Master's degree in a related field is required.
• Active Illinois licensure as LCSW, LCPC or LMFT is required.
• Strong clinical assessment, treatment planning and documentation skills.

Responsibilities

• Conduct assessments and diagnostic evaluations to identify mental health needs.
• Develop and implement individualized treatment plans based on client goals.
• Provide individual and group psychotherapy using evidence-based practices.
• Facilitate group therapy sessions when appropriate.
• Maintain accurate clinical documentation in compliance with standards.
• Collaborate with nursing staff and healthcare professionals to coordinate care.
• Support residents dealing with depression, anxiety, grief, and adjustment issues.
• Promote emotional wellness and improve residents' quality of life.
