Community Physicians seeks a Psychotherapist (LCSW) for an on-site, full-time role in the Western Suburbs of Illinois. The clinician will provide mental health services to individuals, couples, and families.

Responsibilities include conducting psychological assessments, developing treatment plans, and providing psychotherapy sessions, with collaboration among a multidisciplinary care team to ensure integrated, patient-centered care for older adults in skilled nursing and post-acute settings.

Qualifications

• Master’s degree in Social Work and an active LCSW license.
• Strong skills in Psychology and Mental Health.
