Job summary

InnovAge is seeking a Recreation Specialist II to plan, organize, direct, and participate in comprehensive activity programs tailored to frail and elderly participants at our PACE centers.

You will evaluate functional status, perform assessments, and develop recreational programs aligned with care plans. Join a multidisciplinary team to support patient-centered care and community engagement while ensuring accurate documentation and program delivery.

Qualifications

• Bachelor’s degree or Associate degree with field-related experience.
• Two years of social or recreational program experience with frail/elderly within last five years.
• One year minimum working with frail or elderly population.

Responsibilities

• Plan, organize, and direct recreational programs for frail/elderly participants.
• Assess participants' functional status and document progress.
• Coordinate with multidisciplinary teams for Plans of Care in InnovAge PACE.
• Provide adaptive equipment and supplies for recreation activities.
