Job summary

The Savoy at Fort Lauderdale is seeking a Speech Language Pathologist (SLP) to join our rehabilitation team in Fort Lauderdale, FL. The SLP will assess referred patients, develop individualized treatment plans, and document progress to support optimal rehabilitation outcomes.

Qualified candidates hold a Florida license, have experience in long-term or geriatric care, and are proficient in documentation, evaluations, progress notes, and discharge planning.

Qualifications

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of FL
• Experience with long-term care and/or geriatric care is preferred
• Well versed with documentation writing
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Responsibilities

• Assess referred patients to determine rehabilitation needs and capacity for care.
• Develop and administer a treatment program with specific goals.
• Set realistic goals based on patient tolerance and physician direction.
• Document and record patient condition and progress.
• Educate patients and families in appropriate therapy methods.
• Coordinate with multidisciplinary team to ensure integrated care.
