Job summary

H2 Health is seeking a Speech-Language Pathologist for 1:1 patient care in senior living communities. This PRN role involves 4–10 visits per week to evaluate and treat a geriatric caseload with dysphagia, cognitive impairment, and neurologic conditions.

The therapist will work independently and as part of an interdisciplinary team to tailor therapy to each patient’s functional needs. New graduates welcome; active CCC-SLP license is required.

• Graduate of an accredited Speech Language Pathology program.

Responsibilities

• This position will evaluate and treat a primarily geriatric caseload 1:1 with a mix of dysphagia, cognitive impairment, neurologic disorders, and problem-solving programs.
• The therapist must be able to work independently and in teams and make appropriate decisions based on each patient's functional needs.
