Job summary

Qualifications

• Bachelor’s degree in social work or related field meeting state/federal requirements.
• At least one year of long-term care experience preferred.
• Specialized knowledge and any licenses required by applicable regulations.

Responsibilities

• Provide in-service education on residents’ rights, advance directives, and the psychological aspects of aging.
• Facilitate clear, supportive communication among residents, families, and facility staff.
• Maintain complete, accurate documentation per CMS, NCDHHS, DFS, and DMA requirements.
• Participate in interdisciplinary care-planning meetings to support resident-centered care.
• Guide families through medical review procedures and appeals.
• Attend workshops to stay current on best practices in geriatric care.
• Coordinate and facilitate Resident Council meetings to encourage participation.
• Provide guidance about Medicare, Medicaid, and other assistance programs.
• Maintain current resource file on community health services.
• Supervise students on fieldwork and provide a learning environment.
• Communicate with families to involve them in resident care and updates.
• Advocate for residents’ rights and needs within the facility.
