Job summary

The Hallmark in Houston, operated by Brazos Presbyterian Homes Inc., seeks a compassionate Social Worker to support residents across the continuum of care. You will assess needs, contribute to the care plan, and coordinate with families and the interdisciplinary team to promote social well-being and resident rights.

Responsibilities include admissions support, discharge planning, counseling, and liaising with community agencies.

Qualifications

• Must be able to read, write and follow written and verbal directions.
• Experience in long term care setting.
• Ability to work with residents, families and interdisciplinary teams.
• Strong communication and documentation skills.

Responsibilities

• Welcome new resident and establish a trusting relationship.
• Enter initial assessment notes within 72 hours of admission.
• Contribute to the interdisciplinary Resident Care Plan Conference.
• Coordinate care plans meetings and notify residents, family, IDT members.
• Write progress notes at admission and quarterly, or when there is a notable change.
• Initiate and participate in the discharge plan with input from multiple disciplines and family.
• Participate in post-discharge planning as needed.
• Provide active listening and counseling to residents.
• Attend Resident Council meetings and liaise with Activity Department.
• Contact community agencies for resident referrals as needed.
• Assist residents with participation in programs and advance directives.
• Advocate for resident rights and liaise with families, facility and community agencies.
• Meet with nursing consultants as needed and support family events and programs.
