Qualifications

• Bachelor’s degree in Social Work (BSW) or related field.
• Valid state licensure as LSW or equivalent.
• Proven experience in a skilled nursing or long-term care setting.
• Strong interpersonal and communication skills.
• Knowledge of healthcare regulations, Medicaid/Medicare policies, and community resources.
• Excellent organizational and documentation skills.
• Ability to work collaboratively within a multidisciplinary team.

Responsabilités

• Conduct psychosocial assessments of residents to identify needs, preferences, and support systems.
• Develop and implement individualized care plans in collaboration with residents, families, and healthcare teams.
• Provide emotional support and counseling to residents and families.
• Coordinate with community resources and service providers to access services.
• Advocate for residents’ rights and ensure compliance with regulations and policies.
• Document interactions, assessments, and care plans accurately and timely.
• Participate in interdisciplinary team meetings to discuss resident progress and care strategies.
• Educate residents and families about resources, rights, and health management.
