Job summary

Neighborhood Healthcare seeks a Social Worker for the PACE program to manage a caseload, collaborate with the interdisciplinary team, and improve health and quality of life for senior participants. This on-site role supports family and community engagement in Riverside County.

The position requires a Master’s in social work, at least one year of elder care experience, and certifications (BLS, CPR, First Aid). Bilingual English/Spanish preferred.

Qualifications

• Master’s degree in social work required.
• One year of experience working with the elderly population required.
• Current Basic Life Support (BLS) certification required upon hire and must be maintained.
• CPR and First Aid certification required for all PACE employees.

Responsibilities

• Manage a caseload for PACE participants with the interdisciplinary team.
• Provide participant health status updates in daily meetings.
• Obtain extensive psychosocial history from participants and/or family at admission.
• Coordinate family conferences addressing care levels, non-compliance, and disenrollment topics.
• Develop and facilitate group counseling for at-risk adults 55+ with chronic conditions.
• Coordinate with participants, families, and providers for advance life planning and end-of-life resources.
• Conduct home visits and assessments in conjunction with the PACE IDT as needed.
