Job summary

myPlace Health, a PACE organization backed by SCAN Group, seeks an MSW to plan, organize, and deliver social work services on-site at our Los Angeles center. You will maintain a caseload, collaborate with the IDT, and support participants to live independently at home.

Role requires MSW, CPR/First Aid, 2 years in psychosocial assessments and care planning, and California driver’s license. Bilingual abilities are a plus.

Qualifications

• MSW required.
• CPR and First Aid certification required (or within 30 days).
• 2 years of psychosocial assessments, care planning, and case management experience.
• 2 years of experience with frail or elderly populations.

Responsibilities

• Engage in daily IDT meetings and conduct psychosocial assessments.
• Present assessments at care plan conferences.
• Coordinate care transitions between hospitals, homes, and facilities.
• Connect participants and families with community resources and support.
