Erickson Senior Living is seeking a full-time, on-site Social Worker for Greenspring Geriatric Continuing Care in Springfield, VA. You will support residents across skilled nursing, long-term rehab, assisted living, and memory care, with weekend coverage required.

This role emphasizes psychosocial assessments, discharge planning, care coordination, bereavement support, and regulatory compliance in a senior living setting.

Qualifications

• Social Work License in Virginia preferred.
• Social Work Degree required; MSW preferred.
• Minimum 3 years’ experience with older adults; 2+ years in Skilled Nursing/Long Term Care.
• Experience with EHR/EMR preferred; proficiency in Microsoft Office.

Responsibilities

• Complete psychosocial assessments and required documentation per policy and regulations.
• Coordinate Social Work from admission to discharge and support transitions.
• Contribute to care plans with multidisciplinary team and family involvement.
• Provide bereavement support to residents, families, and staff.
• Monitor psychosocial and medical social needs and document in medical records.
• Maintain compliance with applicable regulations and professional standards.
• Identify and resolve grievances and support resident-centered decision making.
