You will advocate for residents' well-being, conduct psychosocial assessments, and collaborate with families and the care team to address social, emotional, and environmental needs in services that promote quality of life. The ideal candidate holds a Master’s degree in social work, active state licensure, and experience in long-term care or geriatrics.

Qualifications

• Master’s degree in social work from an accredited program.
• Current licensure or certification as a Social Worker in the state of employment.
• Experience in Long-Term Care, geriatrics, or related field.

Responsibilities

• Conduct comprehensive psychosocial assessments to identify needs, strengths and preferences.
• Collaborate with residents, families, and the interdisciplinary team to develop care plans.
• Provide counseling and emotional support to residents and families.
• Advocate for residents' rights and preferences in care planning.
• Connect residents and families with community resources and advocacy organizations.
• Provide crisis intervention and support during challenging situations.
