Qualifications

• Master’s degree in social work from an accredited program.

• Strong assessment, counseling, and crisis intervention skills.
• Excellent communication and interpersonal abilities.
• Ability to work collaboratively as part of a multidisciplinary team.

Responsibilities

• Conduct comprehensive psychosocial assessments to identify residents' social, emotional, and environmental needs.
• Collaborate with residents, families, and the interdisciplinary team to develop individualized care plans.
• Provide counseling and emotional support to residents and families, offering guidance on coping strategies and transitions.
• Advocate for residents' rights and preferences in care planning and decision-making processes.
• Connect residents and their families with community resources, support services, and advocacy organizations.
• Provide crisis intervention and support during challenging situations.
